Complete Required Forms

The following forms may be required and should be uploaded as PDFs during submission:

  • Application to Construct or Demolish form is required for ALL submissions.
    • If you are not the property owner, as identified on the property deed, we will require the owner’s authorization. This is requested on page 5 of the application form.
  • Schedule 1: Designer Information is required for ALL submissions, unless the construction drawings were prepared by an architect or engineer.
    • The individual who prepared the construction drawings will need to complete this form, to take responsibility for the design.
      • BCIN designers will complete the form, indicating their level(s) of qualifications and their BCIN, which will be verified by the Permit Clerk via the QuARTS Public Search Registry.
      • If a homeowner has provided their own construction drawings, they will need to complete this form.
    • For tent projects;
      • Any individual can take responsibility for a tent to meets the following criteria:
        • the area of the tent is greater than 60 sq m (645 sq ft);
        • the aggregate area of all tents in a group is greater than 60 sq m (645 sq ft);
        • the tent is attached to a building; and
        • the tent is to be erected less than 3m from other structures.
      • A qualified designer will need to take responsibility for a tent that meets the following criteria:
        • the area of the tent is greater than 225 sq m (2,422 sq ft);
        • the tent contains bleachers; or
        • the tent is enclosed with sidewalls.
  • Commitment to General Review by an Architect and Engineer is required for projects indicated in Division C, Section 1.2.2. of the Ontario Building Code.
    • Typically, this form is required for most non-residential projects.
    • Required for demolition permit applications where:
      • Buildings that exceed 3 storeys in building height or 600 m² in building area,
      • Buildings where the building structure includes pre-tensioned or post-tensioned members,
      • Buildings where the demolition will extend below the level of the footings of any adjacent building and occur within the angle of repose of the soil, as drawn from the bottom of such footings, or
      • Buildings where explosives or a laser are to be used during the course of demolition.
    • Required for farm buildings:
      • is more than 600 sq m (6458 sq ft),
      • falls out side the prescriptive provisions of the National Farm Building Code on the standard requirements of SB-11 or,
      • is a liquid manure storage tank
    • Required for a permanent sign
      • where the sign is, a ground sign that exceeds 7.5 m in height above the adjacent finished ground, a projecting sign that weighs more than 115 kg, a roof sign that has any face that is more than 10 m2. or a projecting sign   
      • attached in any manner to a parapet wall.
    • Required for a temporary tent
      • the anchorage and structural system of all tents greater than 225 sq m
  • Schedule 2: Sewage System Installer Information is required for all septic system submissions.
    • The individual who prepared the septic design will need to complete this form, to take responsibility for the design.
      • BCIN designers will complete the form, indicating their level(s) of qualifications and their BCIN, which will be verified by the Permit Clerk via the QuARTS Public Search Registry.
      • If a homeowner has provided their own septic design, they will need to complete this form.
  • Residential EED Summary: SB-12 is required for all new residential construction.
    • The individual who prepared the heat loss design will need to complete this form, to take responsibility for the design.
    • This form, in conjunction with a heat loss design, will need to be provided when residential additions exceed 20% of the floor area of the existing house.

    • NECB 2015 + SB-10 Div.3 and Chapter 3
    • ASHRAE 189.1-2014
